Ontario Health Teams
The Ontario government is building a connected health care system centered around patients, families and caregivers. These changes will make it easier to navigate the system, strengthen local services and allow health care providers to work together to take the guesswork out of transitions.
Timelines for Ontario Health Teams have been updated and the received submissions are being reviewed by the MOHLTC. An Ontario Health Teams webpage has been created, and is the best place for you to stay updated with information, resources and timelines.

The Connecting Care Act, 2019
On June 06, 2019, the provisions of The Connecting Care Act, 2019 came into effect. This new health statute was introduced under Bill 74, The People’s Health Care Act.
The Connecting Care Act, 2019 lays the foundation for the continued implementation of Ontario’s phased strategy to transform and strengthen the public health care system. Several other Acts were amended to align with The Connecting Care Act.
